In order to successfully pass a work term, students are required to develop work term learning objectives, participate in a site visit, and prepare a work term assignment. We also gather feedback from you about the student and the Program. Final Co-op Evaluation Form – To be completed at the end of the work term. If students are doing 2 or more consecutive work terms with you, this form is only completed at the end of the final term.
Assignment Release Form – Co-op students are required to submit a work term assignment to the co-op program for each 4 month co-op work term. Before submitting these assignments to the co-op office they should show you what they have produced and obtain confirmation from the employer that their assignment does not contain confidential information.
Work Term Assignment Evaluation Form – For work term assignments containing confidential information, the employer must provide complete and return the Written Report Evaluation form:
